I am unable to find any subscription associated with the email you provided.
You can purchase a W3 Total Cache Pro again and use the standalone checkout for the bulk of 5 activations, and we can refund the initial purchase.

However, since this is a public forum, please send the information about the initial purchase to Performance>Support>Billing via the plugin so we can check this subscription and provide a refund once you have completed the purchase for 5 Site Licenses.
