Hello Corey,
Thank you for contacting us about adding a PDF/Word Form for a service. I recommend using WPForms since it is included with BoldGrid, and has pre-made forms you can easily use.
This will email you the information that is entered. There may be 3rd party options available for using PDF of Word, but I was unable to locate any popular ones.
